Dead oto!

I have had 3 otos in my planted 10g for almost two months. All water stats are normal. All were seemingly healthy until last night--I found one little guy dead. When I pulled him out, I noticed he had a dark green bell and abnormally red gills. Any clues to what could have been the problem?
 
Red gills is usually a sign of ammonia poisoning.

What were the actual readings of your tank?

Ammonia
Nitrite
Nitrate
pH


Oto's are usually quite delicate initially due to stress as most Oto's are wild caught.
