How do I get Planaria out of my aquarium?

I have little worms floating in my fish tank that I noticed about a week ago, they are floating everywhere, I've been doing water changes and it doesn't seem to do anything, any advice would be great, thanks

Do several gravel vacs. That should get rid of them. They show up when the substrate has to much uneaten food, and whatnot in it.
