Java Fern Disease ??

Any one have any idea what this is and what causes it?
It started on one fern about 2 weeks ago and now it spread to all of them in the tank, even on the opposite side.

Looks like you'll soon have a bunch of baby Java Ferns growing on those leaves.
 
Once baby plants grow from leaves, the parent leaves die. If these are the same leaves that those baby plants grew from, then go ahead and remove them.
 
I'm leaning toward an imbalance in nutrients and/or lighting. I'm probably not going to be able to give you any answers but if you give us info on the tank perimeters someone may be able to help.
Lighting - type, watts, duration of light, sunlight or not
have you tested the water? LFS can do it if you don't have test kits. Make sure you tell them the tank has plants.
Is it light, moderate or heavily planted

are you using: ferts (details) carbon source (like florish excel) carbon or ammochips
