Plant been in tank for 6 weeks , noticed yesterday black brown on stem is plant dying ?

I saw it most often in cabomba and lloydelia but when the lower part of some plants doesn't get enough light the stems can melt away allowing the plant to be moved to a place that is more suited to sustain it. It's not so much dying as it is relocating.
 
Leaves are dropping off aswell

Each stem needs to be planted individually with the leaves of each stem almost touching the leaves of the stems planted around them. Yours are in a bunch which doesn't allow light or water carrying nutrients to reach lower leaves. The best thing to do is to cut off the bad bottoms and replant the healthy stem stops as indicated above.
