QT tank question

So I've been thinking about a QT tank but am wondering a couple of things.

I guess the basic one is how? If the tank is only to be used for new fish or sick fish how do you maintain the perams/care? Without anything in the tank how do the BB stay maintained? Obviously you don't want to be dropping all new/sick fish into an uncycled tank.

Or do you just keep something like an already established filter on a current tank for it and set up the tank to instant cycle it when you need it??

I'm sure this is something that should be completely obvious to me, lol.
 
I have a sponge filter running in my 29-gallon tank, in addition to the AquaClear filter. When I need a QT, I just move the sponge filter over to it. Other people keep a sponge or other piece of filter in their regular tank and transfer it to the QT--same concept.
