Is this a planted tank? Any diggers in your fish population? It can be difficult to maintain a multi-layer substrate.
 
Well, I don't know if it will help your problem, but it won't hurt anything. Be aware that your sand may be a big part of your biological filter, so only put down a thin coat each week until you're happy with the new substrate depth. This way the new sand can start to take up the bioload without completely smothering the old sand before it's picked up the load.

You'll have to consider the substrate carefully if you ever need to re-arrange your plants. With these particular layers, you do have the advantage that larger particles naturally move to the top...
