Are Sally Lightfoot collected or bred in captivity? And if they are collected, is it done in a sustainable manner?

I imagine they are collected, most marine critters like that are.
Some corals and a few dozen fish are captive bred, but a lot of those species are still collected as well.

Whether or not it is done in a sustainable fashion is dependent on the scruples of the collector.
There are very strict laws in place and quota's imposed on collecting wild animals, so if the collector is working in accordance with the law, fine, if not, who knows.

I suspect they are collected as they are probably tedious to breed, like most crustaceans in our hobby. They are probably too easy to catch to even attempt to breed them for lucrative reasons. Even if people bred them, the captive bred ones would probably cost much more.
