Unhealthy Aquarium?

I have a 40 gallon reef aquarium. It has been set up for almost a year. It has a couple hardy corals, along with what was 2 fish.
My tests all come out perfect (ammonia, nitrite, nitrate, phosphate, calcium, ph, dkh).
The algae has always been terrible, usually green hair algae, but also red slime. It has taken over 2 corals!! I have a pretty large clean up crew.
What really worries me is, I found my royal gramma dead today! What's going on?
 
High nutrient level...as in dissolved organics. Do you run carbon, and/or GFO and do you have a protein skimmer (if so, what kind)?
 
I agree, you have a high nutrient level in the tank, which is feeding the hair algae. I would suspect part of the problem would be over feeding.

As was already asked are you running a protein skimmer? It is a great way to remove dissolved organics in the tank, in addition to regular water changes.
