Detritus or planaria?

Hello everyone, years ago I have had worms in my tank and somehow the problem took care of itself, I now have a 15 gallon shrimp/beta tank set up, just recently I added a large eucalyptus root (boiled for hours) and an assortment of plants, I assume the worms came from the plants or wood, so my first question is how do you get them out of your tank and and my second question, is it detritus or planaria? Also my tank is cycled and tests clean with API master kit.
