Everything is 0 and the p.h is 6.5. Then the tank mates are 2 flame tetras,2 Danios and 1 sunset gourami?

I also put a sponge on the intake and I think it's getting worst. I put aquauim salt and melaflex but it's still not getting better?

Could be fin rot. Personally I think you should start treatment although it being a discus I don't know if it would be safe. I'm not a discus guy but I do believe it may be fin Rot.

Are you able to isolate him? He may get better Juarez with rest and lots of clean water. Have you noticed him being harassed by the other fish?

'Just with' clean water I also use stress guard- its said to protect and help them to heal.

Firstly everything being at 0 is 9/10 a sign something is wrong, nitrAte should be above 0 except in heavily planted tanks.

Secondly if it's occurred quite quickly it is likely to be the result of other fish attacking/fin nipping your discus.

What tank size are we dealing with?
