Just did a 20-30% water change sucking out as much as i could
And replaced the carbon cartdridge with another sponge since it doesnt do anything

The worms are probably planaria. They eat leftover nutrients. Have you tried water changes at all?

EDIT: Alright, then, what are your paramaters?

How old is the tank? Diatoms are largely unavoidable in the first two months or so of the tank.

Probably like 3-5 months i cannot remember but it has been around since the beginning of the tank

Is that fluval stratum? It might just be dust from that.

What kind of light do you have on the tank and what size is the tank?

You've probably get fairly significant light. Hopefully the CO2 will help your algae situation.

Ya same here :( also noticed its only on my slow growing plants. My bacopa japan, rotala r and hemanthius umbrosum are all good. But not so for my java ferns anubias, and crypt
