The darker patch in the white area is a deeper hole. It's like the flesh is necrotizing off his face.
You can see in this pic how buggy his right eye is
I keep considering just euthanizing him, but we have 3 more days of antibiotics to go still.

Did your tank levels end up balancing out and it was just he was too far gone or was your water just doomed no matter what you did

I think he was just too far gone, although I was doing water changes every day. Google later told me that Maracyn 2 kills the bio filter, so that's some explanation there.

my biggest worry right now is my main tank is a bit unwell, but that's another topic.

As for the hospital tank, at least it is available now for my single mystery fry to grow (after a very thorough cleaning)
