Leather/tree turning yellow/green

#1 I need an ID on this coral, and #2 a possible explanation for discoloration.

It used to look like this

But after I moved it a couple times in a week (which it usually will recover from) and then switched out the lights from the stock lamps (8 x T5HO Nova Extreme) to ATIs and Geisemann lamps, it turned pretty much white on the top. The bottom has almost always stayed "brown" (extended polyps) but now the top seems to be almost always "contracted" and looks more yellow/green than white. It has been this way now for a couple months.

Any suggestions, advice, or insight would be helpful.

Temp 75-77
Salinity 36
Ammonia 0
Nitrite 0
Nitrate 0
Phosphate 0.12 (and has been that way for a year)
KH 8 ish
Calcium varies 410-450 throughout week
Magnesium 1180 ish
Lights are 4' T5HO 5xATI Blue Plus 2x ATI Purple Plus 1x Geisemann Aquablue. 4 BPs on 7am-4pm, rest are on 8am-3pm
Tank is 125 w/250+ lb LR established running 3 years+
Filter is ATS running 3 months
 
Mine went white when I had a phosphate and cyano problem,it also shrunk alot but is gradually coming back now my lighting is back on longer.
