Toadstool coral sick

Does anyone know what is wrong with my toadstool coral? I have the picture of it.

The root of it is brown and weaken and it is not standing straight up and leaning on the side.

How long has it been like this? Can you tell is more about the tank, size, lighting, current parameters, how long you've had it?

I had it for about 2 weeks, just noticed it today. 50 gallon rodi water with ammonia 0, nitrate0, nitrite 0. I did however added some coral nano 2 steps from petco that I brought 2 days ago. It suppose to give corals all the nutrition it needs.

Led? T5ho? How many bulbs? More info you give the better this can be assessed.
It could simply be stressed and shedding its slime coat.
