Ich is pretty easy to diagnose. Obviously the white spots all over the body are a big sign.

I wouldn't say it's impossible that heat treatment is already doing its magic helping it pass.

The white spots fall off the fish and then reproduce and then release a bunch of free-swimming parasites. The heat treatment should interfere with the reproducing part, but it's why you no longer see those spots.

Some of the free swimming stage that was around before you started treating may have already reached a host and may form more cysts ("white spots")- hence "it gets worse before it gets better", but yes, it's possible that the treatment is already taking effect.
