Ich meds 4 wks, now on general disease meds

Dear enthusiastic fish people,

Should I continue the ich routine with now a change in meds?
We were on ich meds for 3 weeks, doing the gravel cleaning every 2 days, change 1/3 water every 3 days, add 5ml ich meds, heater on 29, no plants and no carbon filters.

Now, we're off ich meds and trying a general disease meds that says, change water and put active carbon in after 7 days.

So, I shouldn't clean the gravel for 7 days? But what about the ich eggs?

Yes there's spots on the fins, not bad. Also patches of white where the scales have fallen off.
Indicates a bacterial infection bc not much change with ich meds.
Big koi is lying on ground too much with his head in the corner not being himself.

What’s your water temperature? Generally ich should be gone (assuming successful treatment) after say 10 days for sub-tropical / tropical (shorter life cycle times for higher temperatures).
