Looks like a tiny fuzzy white cotton ball on his nostril and one inside...he is eating, playing, eating his plants, swimming all over his 55 gallon tank, he was treated for columnaris couple months ago..quickly got better...now this...THANKS IN ADVANCE! love this guy...his name is Ari !!

This might help you out, it seems to be running rampant for people right now:
Columnaris (Flexibacteria); Aquarium/Pond Treatment, Prevention; Fungus, Saprolegnia
If not, it could be a bacteria. What is your water changing schedule? What is the tank size? Do you know your water parameters?

It's 55gallon tank..by parameters do mean the nitrates, ph,ammonia levels..I'm not sure...my nitrites were at .5...did a water change week ago...cause my two fish were in QT treated with jungle lab fungus clear...while the other tank(55gallon) still was running and ...so I kept up with the PWC. now after one fish died...I just replaced the other back to tank...
