Problem with betta's gill

We have a betta that has what looks like a swollen gill. It's only on one side and the other one is fine. He's able to swim just fine, but he chooses to rest at the bottom, unmoving and doesn't respond to us coming to the tank. He's very pale and washed out, still responds to food but it looks like his interest is waning and it takes him awhile.

This has been going on for about a month or so now and he's not getting better. Two weeks ago we had treated his tank with general cure to get rid of some white worms in his tank but that didn't work.

The water parameters:
Ph: 7.4
Ammonia: 0ppm
Nitrite: 0ppm
Nitrate: 5ppm

He's in a 10 gallon tank and we do water changes once every week. We're just getting worried and we dont want to treat him with any unnecessary meds.
