Sick swordtail

Hi guys,

Any ideas what this could be going on with my swordtail? She has a white spot under the scales just behind her head. Looks like the scales in that area are coming loose.

Water: 0 Nh4, 0 No2, 5-10 No3, KH 6-7, GH 12, PH 7.5, Temp: 78F
Tank: 33G, 30% water change every week
Filtration: Eheim Pro 4+ 250
Light: 2x24W T5 6500 K, 8 hours a day
Fertilizer: 3ml EasyLife Profito every other day
CO2: EasyLife EasyCarb 3 ml every day

Probably just a injury. not surre

Could very well be. I noticed the upper side of her mouth looks injured. As if a small piece is missing.
