Troubling Tiger Barb Trend

I've had tiger barbs for going on two years now and have noticed an unfortunate trend with my group. It seems that the group will all be doing well for a while and then all of a sudden one breaks off from the group and dies shortly their after. This repeats with one or two more (each breaking off individually) until things stabilize.

I have no idea why this is happening as my parameters consistently test well (see below). Any ideas would be appreciated.

36 Gallon
CO2 Injected
Ammonia - 0
Nitrite - 0
Nitrate 5-30
pH - About 6.5
Tiger barbs - 8-10
 
Do you know your tank water hardness and or TDS?

Do top off your water consistently with Tap water or RO?

How much water do you change on a regular basis? Size of tank?

Do you notice any of these deaths before regular maintenance, after water changes?

How often do you do pwc and how much do you usually change?

Were these adult fish when you got them?

You have BB in your filter pad, don't change the filter pads?
 
Also does one get picked on? I’m finding one seems to get picked on and eventually goes, then the next one (too expensive to just keep buying them in hopes they settle into a pack).
