Snails, although, you'll want to keep the powerhead clean manually. Snails + powerhead, can easily = dead snail.

Diatom algae is a natural part of a tank, it is fueled by silicates in the water.

So snails and some scraping. Will it hurt if I leave it on the back and sides? Just clean the front? Thanks

Nope won't hurt at all, just a little unsightly.
