I've purchased live fish and a cleanup crew from liveaquaria.com and was extremely satisfied with their livestock and service. They are very professional and the live items were extremely well packed.

I've also purchased some livestock and most of my equipment from fishsupply.com. I haven't really seen anyone else mention them on this board, but i'm really pleased with their service and their shipping costs for livestock seems to be a bit cheaper than other places. www.fishsupply.com is great for me because they're located in Los Angelos and I live in Northern Cali so I usually get my orders the next day.
