Looks like you have that anubias' rhizome planted, it will likely rot away if it's under the gravel. The roots are fine, but the rhizome must be above the substrate.

What are your lights? make/model/colour temp?
How deep is the tank?
How long are the lights on?
What is your fertilizing/water change schedule like?

Most likely a lack of fertilizer.

You have a couple of things going on, the rhizome on that anubia looks buried that will kill it. A lack of fertilizers for the sword and diatoms to top it off. I am guessing it's a newly set up tank?
