Help identifying this worm??

We got a couple new live plants a few days ago and saw this little white guy crawling on our tank glass! What is it? A shell-less snail? A worm?

Does the head of the worm look a little on the triangle side with two dots that resemble eyes? Does it appear to be round or flat? If the latter and you answered yes to the first questions, then it's most likely planaria.
 
It was so tiny I couldn't tell the head from the tail, and it did not look flat
