Holes in leaves?

Any ideas on why this is happening?

Water parameters are pretty ideal...

I'm also having the same issue with a very similar plant. I'm wondering if I have a pond snail I can't find yet.

If it's only on older leaves then it could be the natural dying off which allows pests such as pond snails to eat away. Once a hole is opened my RCS nibble at the edge making it bigger. They don't touch healthy leaves.
I remove all leaves with holes and hunt out unwanted pond snails.
