Noticing some life on it, looking like some worms poking out and some red clam like things.

Pictures, and I would go to the Identification thread for your answers.
But, worms more and likely are common bristle worms and are a good thing for your tank. I wouldn't worry about them. Need pictures for more accurate identification.
I can't help you with the red clam thing, because I don't know. Need pictures!!
