I would say you added too many fish too fast and had an ammonia spike but your water parameters are good it wasnt ammonia poisoning that killed your fish if your testing it at zero, 5 parts nitrate is not a problem, people that keep planted tanks try for 39 ppm nitrate. It doesnt sound like...
