yep, if you add a substrate you would have to clean it so it's really up to you. you are just creating a mini environment that you KNOW isnt diseased and is also isolated from your other fish, where you can spare them the effects of the medicine, and where the sick fish can recover.

it wont leech iron. lava rock is basically glass, most of the time its compounds resemble granite which also does not react in water. dont worry about it.

bamboo isnt meant to be submerged, it will grow for a bit but soon it will rot.

you want it to be it's own system so yes, running water, either hydro-sponge or just a air stone would do the trick, you dont want the water to become stagnant. the tank size doesnt matter. but usually the more water the more stable your environment will be.

i think a good rinsing, soaking, then re soaking in new water, then a good rinse is all you need. im currently soaking some lava rocks i got from a building material outlet.

best to treat them separately cause medicines can stress otherwise perfectly healthy fish. if you have the space and equipment i dont see why not.

i have red lava rocks in my sump and i never encountered any iron related problems.

i dont think anything grows if it's getting killed, you might want to buy one of those plant protectors that wrap around the stem.
