    I am by no mean an expert, but in my opinion this sounds like "hole in the head disease," scientifically, head and lateral line erosion. This believe it or not is actually caused by using active carbon in your filter! My advance, TAKE IT OUT ASAP! Start doing 50% minimum, up to 90% water...

    Just curious, have you tried testing the water in the fish bag, before you add some of yours? If the levels are quite different than your tank water, like a higher pH, hardness, temp, combination of things, etc. this could at least give you some answers.
