Seeking help. Green Severum with white protrusion.

Green Severum has a white protrusion that looks to be growing. when it was a young fish this was a white bump, i had treated for parasites and it went away. Can anyone ID this issue and what i should do? it is about 3 years old

It does appear to be a worm and worms are common in Cichlids so I would treat with any medication that contains Metronidazole and Praziquantel. ( i.e. Fritz Paracleanse, API General Cure, etc. )
