Is it the placement of my koralia's or just not enough flow?

There should be enough flow at any time to keep detritus from forming. IMO, if you use a wave maker, which I recommend, you may need more fan, or bigger ones to make up for the fact that they don't all run at the same time.
 
I'm getting really good flow now with the timer set to 15 seconds simultaneously. The only problem now is the air bubbles that form where the water crashes.
 
I've changed my PH to have continuous flow and I took off the timer. I'm now getting better flow. I'm planning on buying two 750 koralias and hooking them up to the timer. This should give me a better result.
 
It's hard to have too much flow. I kind of use the rule; turn it up as far as your can until the sand starts moving then back off a notch.
