Is my Star dying?

Hi guys, I hope im posting this to the correct forum, I have a star in my 80g since 2 months now, I saw the star moving around everyday some times on the tank walls , sometimes on rocks etc. but since 4-5 days I saw the star on the substrate and not moving since few days, thought its small legs looks like moving but doesnt move the star anywhere which make me feels it is still alive. But since 2 days I see the star corners are breaking ... attached is the picture ...

Tank is 80G with Live Rocks
Ammonia 0
Nitrite 0
Nitrat < 40
PH 8.3

have you tried to move it to another location like on a rock or something.

Stars have a rough time with higher nitrates. It does not look in good condition there.
