Black moore eye popped out, layer of skin around it

I have 2 black moores that have one of their eyes popped out and a layer of skin around it. I change half the water once a week. Clean the gravel. Have emp 400 penguin 350 and a fluval 305. Powerheads and anacharis. My levels are .25 ammonia 0 nitrite 10 nitrate hr ph 7.4. Temp around 73. I have 3 black moores 2 telescopic eye. 2 ryukin, Any advice. Thanks

I'm really sorry to hear that. what's your tank size? Have you ever witnessed bullying or does it seem like there's any signs of infection?

75 gallon. No they don't fight. I change the water and gravel vacuum too. I feed frozen blood worms and krill. Basically a variety.
