I have been giving my fish Melafix for about four day. ( I have not removed the active carbon). Their bodies look like they have scratches or that the scales have been rubbed off. One has a little hole in the side of his head but I don't think it is hole-in-head, but it might be. I have tried to take pictures but they don't come out at all and the fish get scared. I thought they were just fighting but I don't think so anymore. I am having trouble figuring out what it is. I don't want to give the wrong medicine. A also have a pleco in the tank that seen to be fine. Please help.

Remove the carbon. The carbon just removes the melafix as you put it in. If you don't remove the carbon, you might as well not even put the medication in.
