Commonly it is a result of excessive light in a low (or inconsistent) CO2 environment, although it can also be seen in medium light builds where there are fairly dramatic imbalances or poor tank maintenance.

The best way to get rid of it is to reduce light/increase CO2, and THEN use chemical warfare to get rid of what's already in the tank.
