He looks very dark. Try doing daily water changes of 30%. If the fish is still eating angel foods have some nice medicated food for hexamita ( the holes he has).
Well they don't say anything about nitatrates spikes in the water, but I have read some articles over the Internet, on different brands of pond fertilizers that some people complained about the same thing. That's why I ask ;)

How big is your tank and your fish, Discus need around 10g per adult fish and also need to be in larger groups. Is the one turning dark the smallest? Or is it being chased around by a bigger Discus?
