I've never dealt with petco, I have a big Al's near home who will take back any fish as long as it is healthy and give you a store credit for the fish.
As for the cleaner shrimp I have not personally heard of it helping with ich but doesn't mean it's not true. My yellow tang loves the cleaner...
I have a yellow tang in my 46 gallon. I know I will have to put him in a larger tank or get rid of him eventually as they do require typically a 100G for lots of room to swim however my tang has done very well to date and does not seem stressed at all in the smaller tank.

The first picture doesn't really show the fin but its the small fin on the side. This one might give you an idea anyways as you see the one is longer than the other.

Bit of background, I recently introduced my first fish a week ago in my 46 gallon tank after cycling the tank for the first time. We introduced 2 clown fish and 1 small yellow tang (at the time I was not aware they required a larger tank).
