Carbon..yes or no?

Hey everyone, I've just recently got back into aquariums. Carbon was always used way back when.. Now im finding out that it isn't really necessary. When would be the right time to use carbon, & when would be the right time to not use carbon?
 
seems the common recommendation is that unless you have recently medicated your tank and need to remove the meds there is no real need for it. Someone else may have other times that it's needed but this is the only one I know of

Okay.. So in a filter.. If I don't use carbon, do I just use more media?
