Columnaris Disease

Help my fish friend has this terrible disease I’m treating with kanaplex jungle buddies fungus and Indian almond leaves and lifeguard and aquarium salt dips he’s in a 3.7 heated and filtered tank for about 1 year and his fins are ragged

I think the issue looks mild and should be treatable.
How often do you do water changes and clean filter?

I usually do water changes every other day and I have a sponge filter I have brackish water from Indian almond leaves
