What size tank?
What kind of fish?
How many fish altogether and how many?
How long has the tank been set up?
Is the tank cycled?
What is the ammonia, nitrite, nitrate and pH?
When was the last water change? How much?
What is happening to the fish?

Also, assuming the poor guy doesn't make it, don't flush your fish. If it has some sort of disease, said pathogen could potentially find its way into the waterways. When I loose fish, I either bury them in the yard (if it's a tetra or something like that) or bury them in a flower pot if they're special fish (bettas, and my rams 
).
