Help! Parasite?

I noticed these really small white things on the side of the glass a couple days ago they move around kind of like slugs. Haven't added anything recently so I'm not really sure where that could have come from. They look like parasites though and today I noticed that one of my black phantom tetras has growths on his nose and what looks like a scratch on his side. I've been taking the worm things out with a sponge whenever I see them but I'm not really sure what they are and what I vocab do to get rid of them. If anyone knows anything it would be really help full
 
Could it be planaria? They look like little worm's that move on the glass.

I thought that's what it was too but isn't planaria harmless to fish? I also thought that they are a little bigger than what I have been seeing and swim around in the tank which they haven't been doing and don't look like they are capable of
