Galaxea. Medium to high flow. Low to Medium light. I keep mine near sand bed.
They can get very long sweeper tentacles 4-6"+ so keep it away from all other corals. Try to put it near the glass or somewhere where the flow will push any sweepers away from other corals.

Looks healthy enough just needs more flow. You should not see any skeletal structure with tentacles/heads full extended when it's happy.
