Is there something I am missing?

I lost another fish today, a bloodfin. That makes 4 fish in the same amount of months. 2 bloodfins and 2 skunk cories. I test my water bi-weekly, everything is perfect, 0/0/5. I sometimes have to add nitrates for the plants. I have, as of currently, 4 bloodfins, 4 cories (2 skunk, 2 green). 5 random shrimp, 2 keyholes, 5 black neons, 5 nerites and tons of MTS and ramshorn. Oh, amd a BN pleco. Is this all my fault for not QTing? I have had dropsy, bacterial infections and something with white growths around the mouth (not sure what that is called).

qt is really a must. with out it you dont know what you are introing to your closed system.
