Java Fern help?!?

Everywhere I have read about java fern it says that it is a really hardy plant and will grow in almost any conditions but mines seems to be thinning out and dying, anybody know why/had this problem and managed to fix it?
I have recently started using TetraPlant PlantaMin, hoping that this will solve the problem and will add some during each water change.
 
What lighting do you have? How is it planted?
 
Make sure when you plant it that you're not burying the rhizome or it will decay and you'll lose it.

Mine are currently in low light, buried (lightly) in eco-complete substrate, with co2, and fert dosing at water changes. They are growing rather healthy and starting to reproduce.

I also have a small army of RCS cleaning the heck out of them. lol

Can you post a pic of your tank with detail on the java ferns?
