I really need to know what this is and how to treat as of right now I did a water change and raised the temperature by 2 degrees till I figure out what to do, this is. Planted tank also.

Is that the blask'ish mottling (lesion?) around the belly / anal fin? If it is, does it continue around the other side of fish?

Yes on the black lesion molting and it doesn't continue to the other side.

Would you have any idea on where it started or what triggered it? Could another fish have attacked it?

Assuming nothing water related (eg check ph, ammonia) I would think bacterial infection and treat with furan2.
