I would up your water changes and make sure he is eating well.
Blood worms would be best IMO.
Tank should be 82+ degrees.....

He gets blood worms on the reg. Do you think I should set him up in a quarantine tank or just treat the whole tank? There are 3 other blue rams in the tank and a baby betta. (My friend didn't want it anymore and I had room)

I would just change water and watch the other rams...
Could be he is getting whipped by a more dominant male.
Rams are pretty sensitive so moving to a QT is not a great plan as they need a seasoned tank...
Also 4 rams in any tank can be trouble.I keep single pairs in tanks less then 40b...
