Plant help ! Java Fern looks sick

Hi,
I have a java fern plant that's being growing out of a piece of bog wood and it's suddenly taken I a turn for the worse it's slowly starting to die, any help or ideas on how to rescue it?

yer slightly turned brown and yes holes
It's growing on a piece of Bog wood
 
I've also noticed little brown "fuzzy" like pits growing aswell, pretty confused on What this is?:/
 
Is it just the older leaves? It sounds like its starting to grow new plants from the leaves. That's how they reproduce. They get brown spots on older leaves, then the spots sprout brown roots. Leaves and a rhizome then grow. Once big enough you can pluck the new plant off and tie elsewhere.
 
Kl, done that today scattered some around a piece of DW, hopefully get some nice green java fern :) will let you no how it gets on.

Sounds to me like a normal cycle for java ferns - they start to get kind of straggly and develop spots before new plantlets start to grow.
