Is this java fern? I thought it was when I bought it but it's been 3 months and it's only about 4 inches tall. I know it's a slow grower but I thought it would have grown more than an inch in 3 months.

I heard there is a variety that stays small, is this it?
 
Looks like narrow leaf java fern. Although it's a slow grower, it can grow slightly faster if you had moderate lighting and co2 present. You can use liquid carbon as an alternative and a good liquid fertilizer like seachem flourish.

I have low light and diy co2 on this tank. I also dose it with tropica premium fertiliser
 
Are you planting it or is it tied to something ?

Mine grow well. I use liquid ferts weekly. Mine are under mini CFLs 6,500K

I've tied it to a stone ornament, do weekly ferts and have LEDs over it
 
Be gentle with remaining roots. It doesn't have many. You can always float them until you tie or glue them down.
