Please help me with this issue

I have NO idea what is going on in my tank. I seem to have possibly 2 different issues going on. 1 is I will get 1 fish at a time get this lock jaw issue and slowly die. The other is 1 fish at a time will develop a curvature of the spine. I have a heavily planted tank and have watched this go on and tried to figure it out. I have treated with prazi pro for a 2nd time. Please help me correct this issue.

First up check would be is water ok? No heavy metals or weird ph?
