My oto is acting weird he's really lethargic and when I try to wake him up he swims around frantically and sinks to the bottom. I move him to a hospital tank does anyone know why it's doing this

Also it's mouth is red and damaged I don't think it can suck on to surfaces for some reason what should I do

What he's in right now is distilled water I didn't test this week so I'll get back to you on that although I think the behavior has something to do with the fact that his lower lip is gone
